The goal of rhinoplasty is to improve your overall appearance by subtly altering the size and/or shape of your existing nose, bringing it into proportion with the rest of your facial features by:

What can rhinoplasty not do? It cannot create an entirely new nose or alter other aspects of your appearance, so it is important to have realistic expectations and goals from the outset. What should I expect during the procedure?

Most rhinoplasties can be performed on an outpatient basis under IV sedation; the procedure usually takes between one and two hours depending on the details of each unique surgical plan.

Today’s advanced techniques enable the surgeon to manipulate the underlying structures of the nose through a small incision on the columella (the strip of tissue separating the nostrils), otherwise known as an “open” rhinoplasty. This approach allows the surgeon to achieve more precise results because he can see the internal structures of the nose during the procedure. Because the incision is so small, it usually heals with only minimal, barely-noticeable scarring.

In some cases, however, an open rhinoplasty may not be appropriate and an alternate approach may be required. During a so-called “closed” rhinoplasty, incisions are located inside the nostrils.

A revision rhinoplasty is appropriate for patients who have had a prior rhinoplasty. This procedure is more complex because the surgeon must account for previous alternations to the shape, size, and structure of the nose. Revision rhinoplasty usually require a longer procedure time than a primary rhinoplasty.

What should I expect after the procedure?

When you emerge from surgery, you will likely notice that a splint has been applied to the nose to help it maintain its new shape. Nasal packs or soft splints may be inside the nostrils to support the internal structures of the nose; this packing will be removed the day after the procedure. Most patients experience some minor discomfort immediately after the procedure, as well as swelling and bruising around the eyes and nose. We recommend frequent icing to help reduce the swelling, and suggest patients keep the head elevated as much as possible, including sleeping upright.

At the end of the first week, we can usually remove the splint and take out any sutures or stitches. The bruising should have faded by this point and the new shape of your nose should be visible. However, it is important to be patient: swelling usually persists for three to four weeks. Most patients can return to work and light activities after five to seven days and resume all normal activities within six weeks. It can take up to a year for the nose to take its final shape.
