Fat Transfer to Face: A Permanent Alternative to Filler

See how Dr. Bloom used facial fat grafting and a lip lift to replace years of filler with natural, lasting volume. A real patient. A real three-step process.

When Filler Isn’t the Answer Anymore

There’s a point some patients reach where filler stops feeling like a solution. Maybe they’ve been getting it for years. Maybe the results feel off — puffy in the wrong places, temporary, or just not quite right. For this patient, the answer wasn’t more filler. It was starting over.

Dr. Bloom approached her case in three distinct phases, each one building on the last.

Before and after fat transfer to face — frontal, three-quarter, and profile views showing pre-op, filler dissolution, and three-month post-op results following fat grafting and lip lift procedure by Dr. Bloom.

Step One: Dissolve the Filler

Before any restoration could happen, the existing filler had to go. Hyaluronidase was used to dissolve it, giving Dr. Bloom a clear, unaltered view of the patient’s natural facial structure. What you see in the “Filler Dissolution” photos is her face without interference — the true baseline.

This step matters more than people expect. Layering new volume over old filler can produce unpredictable results. Starting clean makes the outcome more precise and more predictable.

Step Two: Fat Transfer + Lip Lift

With a clear canvas, Dr. Bloom performed two procedures simultaneously: facial fat grafting and a lip lift.

The fat grafting process:

  • Fat was harvested via liposuction from the patient’s abdomen
  • The fat was processed in a centrifuge to isolate healthy, viable fat cells
  • Purified fat was then injected into targeted areas of the face to restore natural volume

Because the fat comes from the patient’s own body, there’s no foreign material, no risk of allergic reaction, and no filler migration. The volume looks and moves like it belongs there — because it does.

The lip lift addressed the vertical distance between the nose and upper lip, creating a more youthful lip position without adding filler volume.

Step Three: Three-Month Results

At the three-month mark, the results speak for themselves. Volume is distributed evenly and naturally. The face reads as rested and refreshed rather than treated. The lip sits higher, the under-eye area looks more supported, and the overall structure feels cohesive.

Is Fat Grafting Right for You?

Fat transfer to the face is worth considering if you:

  • Have been using facial filler for several years and want a longer-lasting solution
  • Are bothered by filler migration or inconsistent results
  • Want volume restoration using your own tissue
  • Are open to a surgical approach with meaningful downtime for a permanent outcome

Results vary by patient. The best first step is a consultation with Dr. Bloom to evaluate your anatomy, filler history, and goals.
